This form is for active clients who’ve had appointments with Prancing Paws Ottawa within the last 6 months.
Clients beyond this time-frame are considered new clients and are required to complete the New Client Intake Form with supporting documentation.

Due to reduced operating hours, Prancing Paws Ottawa is currently accepting a limited number of new client inquiries through professional referral only. Supporting documentation is required with all requests.
